Health effects
Chlorine dioxide is a respiratory and skin irritant at elevated levels, and the EPA sets a relatively low MRDL of 0.8 mg/L to keep exposure well below thresholds for irritation. The compound itself converts to chlorite and chlorate in the body, and ATSDR notes effects on the developing nervous system in animal studies at higher doses.
In hot water it vaporizes and is inhaled, which is the same pattern as chlorine and chloramine. People with asthma sometimes notice it more than free chlorine because of its sharper odor.
The larger health concern around chlorine dioxide is its primary byproduct, chlorite (ClO₂⁻), which the EPA regulates separately at 1.0 mg/L because of effects on red blood cells and developmental concerns for infants.
Where it comes from
How it gets into your water
Chlorine dioxide is generated on-site at the treatment plant (it can't be stored in bulk) and used either as a primary disinfectant or as a pre-treatment to oxidize taste/odor compounds and difficult organics before chlorine is added. Utilities using surface water with high organic content sometimes prefer it because it produces fewer THMs than chlorine does.
Use is much less common than chlorine or chloramine: only a few hundred US systems use ClO₂. Where it is used, it is typically maintained at a low residual through part of the distribution network.

Is it in your water?
Only certain utilities use chlorine dioxide, primarily medium and large systems treating challenging surface water. Your annual Consumer Confidence Report (CCR) will list it explicitly if your utility uses it, alongside its byproduct chlorite.
Levels are tightly controlled and rarely exceed the 0.8 mg/L MRDL because the chemistry is well understood and the equipment is monitored continuously.
The fix
How AquaOx filters it
Chlorine dioxide and its byproduct chlorite are both filtered by activated carbon. The Centaur catalytic carbon stage in AquaOx reduces ClO₂ on contact, and downstream coconut-shell carbon polishes off residuals along with any associated chlorite.
Whole-house filtration matters here because chlorine dioxide is volatile: filtering it at the point of entry eliminates inhalation exposure during showers, not just the ingestion route.
