What it does to you
Health effects
The acute risk from nitrate in drinking water is methemoglobinemia, commonly called blue baby syndrome. Infants under 6 months old lack the mature stomach acidity and the enzyme systems older children and adults use to handle nitrate. In babies, gut bacteria convert ingested nitrate to nitrite, which binds to hemoglobin and prevents it from carrying oxygen. The skin turns a characteristic bluish-gray, and severe cases cause suffocation, seizures, and death.
Bottle-fed babies are at the highest risk because formula made with high-nitrate well water can deliver a single dose well above the safe threshold. If you have an infant in the home and you draw water from a well, you must know your nitrate level before mixing formula. The EPA limit of 10 mg/L (as nitrogen) was set specifically to protect infants.
Beyond infant methemoglobinemia, chronic adult exposure is associated with increased risk of certain cancers (especially colorectal and thyroid), thyroid disease, and adverse pregnancy outcomes including neural tube defects and preterm birth. The cancer evidence has strengthened over the past decade and several public health bodies argue the 10 mg/L MCL should be lower.
Where it comes from
How it gets into your water
Nitrate is the dominant chemical contaminant of US groundwater. The sources are agricultural: synthetic nitrogen fertilizer, animal manure, and concentrated animal feeding operations. Anything that puts nitrogen on the land surface in excess of what crops can take up eventually leaches downward into groundwater.
Septic systems are the second major source. A standard septic system discharges nitrogen-rich effluent into a leach field, and in areas with dense residential development on septic, the cumulative load can drive shallow well nitrate well above 10 mg/L.
The geographic pattern is sharp: the Midwest corn belt (Iowa, Nebraska, Minnesota, Illinois, Indiana, Kansas), California's Central Valley, eastern North Carolina, and the Delmarva Peninsula all have widespread well-water nitrate problems. Shallow wells in sandy soils contaminate fastest; deep bedrock wells take decades to show effects but eventually do.

Is it in your water?
If you are on a municipal water supply, your utility's Consumer Confidence Report lists the running annual average nitrate concentration. Most US cities are well below the MCL, but agricultural-region utilities (Des Moines, parts of the California Central Valley, eastern Nebraska) periodically exceed and have to blend or treat.
If you are on a private well, you must test for nitrate. We recommend testing at least once a year, and more often if you have an infant in the home, if you live near row-crop agriculture or a CAFO, or if your well is shallow. Many state environmental and public health departments offer free or subsidized nitrate testing. Standard well panels usually include nitrate; if yours does not, request it explicitly.
Critical caveat for new parents: if you draw water from a well and you are bottle-feeding or planning to bottle-feed an infant under 6 months, do not mix formula with the well water until you have a current nitrate test below 10 mg/L. Boiling does not remove nitrate; it concentrates it.
The fix
How AquaOx filters it
We have to be direct here. The AquaOx whole-house system does not filter nitrate. Activated carbon is excellent for chlorine, disinfection byproducts, VOCs, pesticides, and most industrial chemicals, but the nitrate ion is too small and too negatively charged for carbon to capture. There is no carbon technology that adequately reduces nitrate in residential service flow.
The treatment technologies that do work are reverse osmosis, anion exchange with a nitrate-selective resin, and distillation. If your well consistently exceeds 10 mg/L, an under-sink RO is the minimum response. If multiple taps need nitrate-safe water, or if the loading is very high, a whole-house anion exchange system is the right tool. Talk to a local water treatment professional who can size an ion exchange unit to your specific water chemistry. We will not sell you a whole-house carbon filter as a solution to nitrate. It does not work and infant safety is too important to misrepresent.
