Health effects
Sulfate is a naturally occurring inorganic salt that gives water a noticeably bitter or medicinal taste above roughly 250 mg/L. Most adults adapt to moderate sulfate in their water within a few days, and at these levels there are no documented health effects.
Above 500 mg/L, sulfate has a well-documented laxative effect on adults who are not adapted to it. EPA has discussed setting an enforceable health-based standard around this level but has not done so. The acute concern is dehydration from diarrhea, which is most serious for infants and people with compromised health.
Infants are the population to watch. Their gastrointestinal systems are still developing and they cannot communicate distress, so high-sulfate water mixed into formula can cause persistent diarrhea before parents notice the cause. If a well tests above 250 mg/L and there is a baby in the home, use bottled or RO water for formula.
Where it comes from
How it gets into your water
Sulfate enters groundwater from the dissolution of sulfate-bearing minerals, primarily gypsum and anhydrite. Wells drilled into these formations naturally produce high-sulfate water, and the Great Plains and Upper Midwest are particularly prone because of their underlying geology.
Agricultural runoff adds sulfate from fertilizers, and industrial sources include mining drainage and fossil-fuel combustion deposition. On chlorinated city water, sulfate is rarely a complaint because surface water sources tend to dilute it and treatment plants do not add it.

Is it in your water?
A persistent bitter or medicinal taste in cold tap water that does not improve with carbon filtration is a strong sulfate signal. Hard water scale that includes a white, slightly crumbly deposit (calcium sulfate) is another clue.
On a well, request a sulfate test as part of your standard inorganics panel. State labs typically report it in mg/L and flag values above 250. If you are buying a property in the Plains states, sulfate should be on the inspection list because it is one of the most common reasons people install reverse osmosis at the kitchen sink.
The fix
How AquaOx filters it
Sulfate is not filtered by activated carbon, KDF, or any of the media stages in a standard AquaOx whole-house system. The sulfate ion is too small and too soluble for adsorption-based filtration to catch it.
The right approach is reverse osmosis at the point of use: install an under-sink RO unit on the kitchen tap to handle drinking and cooking water, and let the whole-house AquaOx handle everything else. RO filters 95+% of sulfate along with the rest of the dissolved-solids load. For very high sulfate (above 500 mg/L) or whole-house filtration, anion exchange systems are the next step up, but they involve regeneration with salt brine similar to a water softener.
